About Social Care Navigation

  

The Social Care Navigation Program helps Medicaid members identify unmet Health-Related Social Needs (HRSNs) such as transportation, housing, and nutrition. After screening, trained navigators provide guidance and connect members to community, state, and federal resources to help meet those needs.

Eligibility Guidelines

  

Screening and Navigation is open to all Medicaid members who demonstrate Health Related Social Needs (HRSNs). 

Eligibility for enhanced services requires managed care enrollment, unmet social needs, and meeting specific population or clinical criteria.

Our Mission

 The mission of the Social Care Navigation Program is to connect Medicaid members with the social and community resources they need to improve overall health and well-being, recognizing that health is not just medical care, but also access to stable housing, nutritious food, transportation, and other essential supports.
